Job Description:

The LPN will assess resident's health needs. develop and implement care plans, and provide ongoing support and monitoring to ensure that our patients receive the best possible care. The successful candidate will have excellent communication skills and be able to work collaboratively with other healthcare professionals.

Responsibilities:

• Assess resident's health needs & develop & implement care plans
• Administer medications & treatments as prescribed
• Document resident's progress & outcomes using electronic medical records
• Collaborate with other healthcare professionals to coordinate care & ensure continuity of treatment
• Communicate with residents and families to provide education and support
• Stay up to date with current research and best practices in nursing
• Participate in professional development opportunities to maintain and improve skill
• Other duties as assigned

Requirements/Qualifications:

• Currrent State License as a Licensed Practical Nurse
• Minimum of 1 year of experience as a nurse, geriatric experience a preferred
• Excellent communication & interpersonal skills
• Ability to work collaboratively with other healthcare professionals
• Strong problem-solving & critical thinking skills
